WordPress as after-sales tool for customer feedback

I would like to do implement the following process in WordPress:

General assumption: A customer Feedback consists of the following fields: Name, E-mail, Foto, Comment, rating, Date

1. In WordPress setup a new empty Customer Feedback and prepopulate Name and E-mail adresse
2. Send off an encrypted Link to the customer via E-mail (can be copy and paste to mailprogram)
3. The customer clicks the link and gets directed (without login) to the prepopulated form and fills out the rest.
4. Submits the form and with submitting the form, the encrypted link becomes invalid

Any ideas how this could be done?
