Simple ‘Survey’ appearing in a Modal Window

Hi Forum

I need a solution which asks visitors to my website a simple Yes/No question through a pop-up modal window and records the answers for me to assess.

I’d like to make the modal window attractive and eye catching. I don’t need the ‘survey’ to have lots of features because it is such a simple requirement.

Do you recommend a Survey Plugin? Or perhaps a Modal Window Plugin in which I embed a third party survey?
