Show specific category items in submit form

Hi,

I’m using contact form 7. I have a front end submit form. In this form I like the user to choose from just a few specific categories, not all.
Also I like to put these categories into some structure. Based on choosing the for example:

Languages (Radio box):
German, English etc.

Level (Radio box):
Beginner, Intermediate, Professional

Skills (Checkbox):
X, Y, Z, A, C, F, O, P etc.

Currently my code looks like this:

Please fill in all information bellow to submit video.<br>
Post Title[text post-title]
Video URL[text post-description]
Tags [text* tag] Use comma to separate tags

Select Categories [select* category]

Insert Text below [captchac captcha-1][captchar captcha-1 placeholder “Insert here”]
