Plugin to publish posts on Facebook and Twitter

I am developing a WordPress Plugin that, amoung other things, has to post selected posts to Facebook and Twitter. Is there a way to use SNAP for this functionality – something like a hook or funktion that I can use within my own plugin?

If not, does anybody know another plugin that provides the functionality for this job?
